Starting Birth Control at Tufts
Getting Ready for Your Appointment!

You have been scheduled for a clothes on appointment to talk to a clinician about starting a method of birth control. We try to make these visits quick and easy! In order to do that, we need your help! You will need to do a little homework ahead of time.

• Fill out the "Birth Control Information Form"
• Get information about the different forms of birth control
  • Read the enclosed pamphlet thoroughly to learn about Birth Control Pills
    OR
  • Go on line to Planned Parenthood and read the links on prescription Birth Control choices to learn about Pills, the Ring, and other options
• Bring to your visit:
  • Your completed "Birth Control Information Form"
  • Your Health Insurance card and your pharmacy card if you that is separate from your insurance card
  • Your credit card (this needs to be on file with the pharmacy before they can fill your prescription)

If you arrive for your appointment without your Birth Control Information Form, and without having read the pamphlet or the web site, we will ask you to reschedule your appointment.
