Information for Students who wish to receive Depo-Provera
Injections at Tufts University Health Service
The Tufts University Health Service, TUHS, provides gynecological care for patients,
including both routine gynecological exams, as well as problem centered care. Whether
your gynecological exams were performed at TUHS or by providers outside the TUHS,
students may choose to receive Depo-Provera injections here at TUHS.
If you have been seen by a provider elsewhere and wish to continue your care at the
TUHS, you will need to provide us with the following information:
- Documentation and results of a gynecological exam and pap smear done within
the past year.
- The results of any other lab work or studies done, i.e., hemoglobin,
urinalysis, tests that rule out sexually transmitted disease, etc.
A request form for release of information from your provider outside the TUHS is
enclosed for your convenience. When the information arrives at TUHS, it will be
reviewed by a clinician and you will be notified of its arrival. You may then make
a thirty-minute appointment with a clinician. The clinician will inform you of any
additional laboratory work recommended. A brief gynecological exam will be required
to insure there are no current problems. Attached you will also find a gynecological
history form. Please fill out and bring it with you at the time of the appointment.
