We will accept a written request with the information below:

• DATE of BIRTH
• TUFTS STUDENT ID#
• YEAR of GRADUATION
• WHAT INFORMATION YOU WANT SENT:
   
  • ENTIRE MEDICAL RECORD
  • IMMUNIZATION RECORD
  • MENTAL HEALTH RECORDS, etc.
     
• WHERE SHOULD WE SEND INFORMATION
• HOW TO REACH YOU IF WE HAVE ANY QUESTIONS
