How To Get a Copy of Your Medical Records
Students frequently need copies of their medical record from our
office. Sometimes they are graduating and will be transferring their
medical care to a new provider, so they want their full medical
record. Other times they just need a copy of their immunization
record. By federal law, we can only release copies of your record if
we have your written and signed consent. Phone calls and
email requests are not considered secure enough when dealing with
confidential medical records. We must have your signed consent in
order to release anything in your records.
If you are on campus:
- Come into our office and the receptionist can give you a release
of information form to complete and sign.
- It usually takes about 1 week for you to receive your records.
If you are not on campus:
- Please send us a written request with the information below, and
sign it.
- Name
- Date of Birth
- Tufts Student ID#
- Year of Graduation
- What information you want sent - entire medical record, immunization
record, mental health record, etc.
- Where we should send the information (we will fax immunization
records, but more complete records we can only mail to you)
- How to reach you if we have any questions
- You may either mail or fax the request to us. Our fax number is 617-627-3592.
