About Tufts CMHS: Making an Appointment When you come to CMHS to meet with a counselor for the first time, you will be given some papers to read and fill out before your appointment. These are described below. If you like, you may print these out, and bring the completed forms with you to your initial appointment. Then, when you check in for your initial appointment, you will be asked to complete a short checklist regarding recent experiences and feelings. This should take no more than 5 minutes. Alternatively, if you prefer, you may come to your appointment 15 minutes early and complete the forms at that time. If you have any questions or concerns about the content of these documents, your counselor will be happy to discuss them with you when you meet. Clients Rights and Responsibilities This is a summary of your rights based on the Massachusetts law called The Patients Bill of Rights. It explains confidentiality and advises how to get the most out of your counseling experience.
