About Tufts CMHS: Making an Appointment

To make an appointment, call 617-627-3360, or drop by between the hours of 9am–5pm, Monday through Friday. We are located at 120 Curtis Street, on the corners of Curtis Street and Curtis Ave, just a half block down from the Tennis Courts.

When you come to CMHS to meet with a counselor for the first time, you will be given some papers to read and fill out before your appointment. These are described below.

If you like, you may print these out, and bring the completed forms with you to your initial appointment. Then, when you check in for your initial appointment, you will be asked to complete a short checklist regarding recent experiences and feelings. This should take no more than 5 minutes.

Alternatively, if you prefer, you may come to your appointment 15 minutes early and complete the forms at that time.

If you have any questions or concerns about the content of these documents, your counselor will be happy to discuss them with you when you meet.

Clients Rights and Responsibilities
This is a summary of your rights based on the Massachusetts law called The
Patient’s Bill of Rights. It explains confidentiality and advises how to get the
most out of your counseling experience. It is for you to read and to keep.
Informed Consent Statement and Confidentiality Policy
This contains a brief overview of what to expect from counseling, our
Confidentiality policy, and provides us with your consent to counseling.
Informed Consent to E-Mail Communications
This explains our e-mail policy and allows you to choose whether or not you
wish to receive email from us.
Student Information Form
This asks for basic information such as demographics, contact information,
and some brief background questions.
